Digital Enablement of Connected Care

Clinical and executive leaders of healthcare organizations are becoming collaborative partners in creating—not just providing—experiences focused on patient-centered care. They are working with healthcare consumers to deliver the right care at the right time with new hybrid service options in-facility, online and at home.

To adapt, healthcare providers need to redesign care experiences with new models that amplify personalization and enable control of care to the consumer with a focus on convenience, information, guidance and choice. Taking care to the home, requires a wide range of connected data and services.

Watch this virtual round table as Australian healthcare leaders explore the opportunity of how to offer a truly connected care experience:

  • What data needs to be connected to enable healthcare organizations to monitor and even predict patient service needs? IoT devices, wearables and field ready medical devices can tell a person’s vital health information such as heart rhythm, oxygen saturation levels, transmit alerts and more.
  • Patients want access to their data that is easily consumable. What is needed to deliver consumer apps that can guide them through their care journey?
  • What digital tools can enable the clinician to provide personalized care in the new hybrid service models?
  • What integration to the care support teams is needed to monitor and respond to remote patient needs?
